Northwest Community Health Center’s (NWCHC) is looking to hire a Complex Care Manager/Registered Nurse (CCM) to support its primary care team. The CCM will work directly with vulnerable and complex patients. He/she will be provide comprehensive care management, care coordination, health promotion, transitional care support, and referral to community or social support services by meeting directly with patients at their home and in healthcare facilities and developing individualized care plans which address the patient’s chronic diseases as well as their social determinants of health.
The CCM will also support the implementation, operationalization, and quality improvement of Medication Assisted Treatment (MAT) in collaboration with NWCHC medical and behavioral health providers through tracking and coordination, community outreach, patient assessment, reporting, compliance, and other activities in support of the service.
The successful candidate will be a graduate from an accredited college or university with an Associate’s or Bachelors degree in Nursing and hold a MT Nursing license. 1-2 years of experience in home and community based public health or chronic disease management, or relevant clinical experience is preferred. Must be able to demonstrate clinical competency, excellent verbal and written communication skills, set priorities, and work as a team player. Salary will be determined on credentials, experience and qualifications.
