Northwest Community Health Center (NWCHC) is seeking a professional, well-organized, team-oriented individual to fill our open Licensed Clinical Social Worker Position.
NWCHC has integrated dental and behavioral health care into our medical clinic to create a team based approach to healthcare. As a member of the integrated care team the Behavioral Health Professional has an opportunity to partner with other providers to encourage and help patients receive the behavioral support necessary to impact their overall health.
The Licensed Clinical Social Worker is responsible for providing brief intervention and consultation in real time to patients in a primary care setting; managing the assessment, treatment planning and episodes of brief goal focused therapy for established NWCHC patients; providing crisis intervention as needed; monitoring mental health referrals to NWCHC’s for patients with higher behavioral health needs; coordinating care for complex patients with co-morbid mental and physical health conditions; functioning as a group visit team health educator; acting as a consultant and resource to members of the care team on skillful and effective interventions with patients with chronic illnesses and mental health diagnoses; assisting with staff training as needed; functioning as chronic care self-management support expert and trainer at clinic sites; meeting minimum documentation requirements for all services rendered in a timely manner; ensuring data collection as appropriate and required; assisting with implementation of an effective Quality Improvement Program; as well as, demonstrating knowledge of the principles of growth and development over the life span.
