Description

Northwest Community Health Center is seeking a full time Care Manager to provide care coordination services for NWCHC patients. 

 

The Care Manager will conduct patient outreach and will function as a member of the integrated primary care team.  He/she will work with patients both telephonically and face-to-face with the goal of improving their individual overall health.  Patients will be identified through multiple channels; and many will have co-morbidities that place them at high risk of future hospitalization and high medical care utilization.  The Care Manager will conduct patient outreach regarding preventive health guidelines.  In coordination with NWCHC clinical providers and nationally recognized benchmarks, the Care Manager will implement population health metrics in an effort to close gaps in care.

The successful candidate will have a minimum of 1 year off experience in a healthcare setting or combination of at least 2 years’ relevant education, training, or experience.  Graduate from an Accredited College, University or Technical school with the degree and/or certification in Medical Assistant or Nursing; or combination of relevant education or certification preferred.   Candidates must demonstrate excellent verbal and written communication skills, and a proficiency with technology. 

 

Wage will be determined on credentials, experience and qualifications.
